BASIC INFORMATION
HOURS: 8:30 AM- 3:30 PM
COST: $90.00 per day unless covered by Medicaid or VA benefits.
WHO MAY APPLY: Any adult 18 years or older who could benefit from the health monitoring, nursing care or recreational activities provided by the center. Typical diagnoses of applicants include: stroke, diabetes, vascular diseases, confusion or general weakness due to age. All persons accepted for participation in Silverthorne will be accepted on a one month trial basis.
WHO SHOULD NOT APPLY: Persons known to be combative or who cannot bear their own weight with a one person assisted transfer.
HOW TO APPLY: Call the center and arrange for a visit either at the center, in your home, hospital, rehab center etc… to begin an evaluation process as to the appropriateness of the enrollment in our program. A report of physical examination completed within the previous 60 days as well as physicians orders for medications, treatments and diet will be required prior to actual participation in the program.
